DOA Claim Form
IF YOU RECEIVE ANY DEAD OR UNHEALTHY LIVESTOCK, PLEASE FOLLOW THESE STEPS:
1. Document the issue
Take clear photos or videos of any coral or livestock that arrived in poor condition. Be sure to include an image of your packing slip (found inside your box). When photographing corals, keep them in water and turn on your white aquarium lights for accurate color and visibility.
2. Contact our support team
Send your pictures or videos to info1@beverlyspet.com, or fill out the DOA form below. Please include any helpful details — such as whether the water was too warm or cold, or if a shipping delay occurred. The more information and images you provide, the faster we can resolve your claim!
3. Wait for our reply
Once we’ve received your documentation, our team will review it and let you know the next steps. You’ll hear back from us within 24 hours (Monday–Friday).
⚠️ Do not discard any dead or dying livestock until we’ve given approval, as this may void your guarantee.
Note: If we don’t receive the required documentation, we won’t be able to honor the guarantee.
